• Initial Consultation Form
  This four-page document asks about your medical and dietary history. Bring the completed form with you on the day of your initial consultation. Answer all questions to the best of your ability—this will help your surgeon perform an accurate and thorough evaluation at your first appointment. It will also be helpful in getting insurance approval for your surgery.
